Position Summary
The Service Coordinator (Midland/Bay Counties) – Residential Services plays a key role in ensuring high-quality residential supports for individuals served by the Arnold Center. This position coordinates services, manages documentation, monitors authorizations, and serves as a primary liaison between individuals served, guardians, support staff, and funding sources.
This role is ideal for someone who is highly organized, detail-oriented, and committed to person-centered services, compliance, and teamwork.
Key Responsibilities
• Coordinate and oversee residential services for an assigned caseload.
• Ensure services align with each individual’s Person-Centered Plan and authorized supports.
• Maintain accurate and timely participant records and master files.
• Complete quarterly progress reports and required documentation for funding sources.
• Obtain, monitor, and maintain service authorizations; ensure accuracy of billing codes and units.
• Utilize WellSky, Vertex, and other human service platforms to review documentation, time entries, and progress notes.
• Monitor staff documentation to ensure services are accurately recorded and compliant with funding and regulatory requirements.
• Communicate participant-specific needs, health and safety information, and service expectations to support staff.
• Provide training and guidance to staff related to participant plans and residential supports.
• Maintain regular communication with individuals served, guardians, advocates, and funding sources.
• Participate in audits, committees, and quality improvement activities.
• Provide direct residential support as needed, including personal care, medication administration, skill-building, and community supports.
• Ensure compliance with federal, state, and agency requirements (HIPAA, HCBS, CARF, WIOA, DOL).
Qualifications
• High School Diploma required; Bachelor’s Degree in a related field preferred (Education, Psychology, Social Work, Human Services).
• Five (5) years of progressive experience in a related field may be considered in lieu of a degree.
• Experience in residential services, case management, or service coordination preferred.
• Strong organizational, time management, and documentation skills.
• Excellent written and verbal communication skills.
• Comfortable working with electronic health records and case management systems (WellSky, Vertex, or similar).
• Ability to manage multiple priorities and meet deadlines.
• Valid driver’s license meeting agency insurance requirements.
• CPR/First Aid certification (or ability to obtain).
• Ability to pass background checks, LEIE screening, and drug screening.
Physical Requirements
• Ability to sit, stand, walk, and move throughout residential and office environments.
• Ability to lift and/or move up to 50 pounds as needed.
• Ability to assist with activities of daily living (ADLs), including personal care, feeding, dressing, and grooming.
